At Prenato, we are committed to protecting the confidentiality and security of the personal information (hereinafter PR) of our customers.
This privacy policy explains how we collect, maintain, use, and protect your collected PII in accordance with applicable laws and regulations.
HOW WE COLLECT AND STORE YOUR PERSONAL INFORMATION
We use various methods to collect your PR: online forms, paper forms, emails, telephone discussions and consultations, face-to-face discussions and consultations, or any other form of communication.
In order to have appropriate access to your PR, we use information and communication technologies to collect and store your PR in accordance with the laws in force in Quebec. We use software that allows the use of personalized access codes to access your PR.
Despite all the protective measures in place, no security mechanism can guarantee complete protection.
PERSONAL INFORMATION THAT WE COLLECT
We collect your personal information in order to provide you with medical services. This information may include, but is not limited to:
Identification information: Name, first name, Quebec health insurance number, postal address, email address, telephone number
Financial and insurance information: credit card information, insurance company
Health information: Weight, height, sex, age, age, medical information collection, medical results, medical forms, imaging
HOW WE USE YOUR INFORMATION
We use your personal information to provide you with medical care or to offer you various services:
Proceed and facilitate the booking of appointments
Proceed with the appointments and services requested
Maintain and update your health information
Contact you for a medical follow-up, appointment reminder or a satisfaction survey
Manage billing and process payments
Your personal information may be used, in an anonymized manner, to improve our services and for training purposes.
WHO HAS ACCESS TO PERSONAL INFORMATION
The following people may have access to your PR:
Employees of our company who need your PR as part of their duties and who have signed a
customer PR confidentiality agreement.
Some medical partners and service providers may access your PII for the purposes of medical follow-ups or laboratory analyses. These partners are governed by the laws in force and by the codes of ethics of their respective professional orders. Our company takes reasonable measures to ensure the protection of your PR with partners.
Depending on the services selected, some PR may be transmitted outside Quebec.
Prenato is committed to not giving, selling, or renting your PR to third parties.
We only transfer your PII with your consent, when required by law, or when there is a serious risk of death or serious injury.
HOW LONG DO YOU KEEP YOUR PERSONAL INFORMATION
Your PR will be kept for a minimum period of 7 years following the last service provided by our company.
RIGHT TO ACCESS AND RECTIFY YOUR PERSONAL INFORMATION
You have the right to access, modify, or delete your PII. You can withdraw your consent to the collection and storage of your PII. However, withdrawing your consent to the collection and storage of your personal information may prevent our company from offering you services. You can get a copy of your file by completing a file transfer authorization form.
